Qualification Criteria For Ibogaine Treatment

Treatment must be completely voluntary.
Candidate must sign informed consent forms.
Candidate must undergo a medical evaluation by Avante GP.
Candidate must submit a completed medical history and depression questionnaire.
EKG & lab tests/reports are mandatory.
Additional cardiac testing may be required if any heart-related history is determined.

DISQUALIFICATION CRITERIA

Severe neurological or psychiatric disorders.
Heart disease or uncontrolled hypertension.
Serious liver, kidney, or gastrointestinal conditions.
Abnormal laboratory test results.
Active HIV, Hepatitis, or Tuberculosis.
Pregnancy.
Known adverse reactions to treatment or medication.
